CQC are asking for feedback from providers on their understanding of the proposed new registration process. The opportunity for feedback closes on Friday 7th October 2011.
An extract of some relevant text from the latest review for registration for CQC - we have italicised areas that we believe are important to consider:
We are listening to what providers tell us about the challenges of our current way of working and will use this valuable feedback as we develop our approach for registering primary medical services. We are also working closely with organisations including the BMA, NHS Alliance and the Royal College of GPs, to make the process as smooth and efficient for you as possible.
This is a collaborative process and we need your help to make sure we work together to ensure the best possible care for everyone.
From 1 April 2013, providers of primary medical services will be required by law to be registered with the Care Quality Commission.
By registering, you give assurance to your patients that you are meeting the essential standards of quality and safety. Registration will also provide a way to address poorly-performing practices
Registration is the process where you to tell us who you are, what services you provide and how you are meeting the essential standards of quality and care. You don’t have to be compliant with all of the standards when you register, but we need to know how you are managing any associated risks or how you plan to become compliant.
Each provider will only need to apply once. Once registered, they must continue to meet the essential standards of quality and safety. Registration should not be a burdensome task, as most providers will already be meeting the essential standards and will easily be able to have evidence of compliance.
The essential standards of quality and safety have been created from the 28 regulations that you are legally required to comply with, which are set out in the legislation governing our work.
The standards focus on the outcomes that patients should expect when they use your services. They cover basic areas of care that most providers are already delivering effectively each day, including:The important thing to remember is that the standards focus on the outcomes experienced by patients, not the policies and procedures your services have.
- How you keep patients informed and involved in their care
- How you make sure their care is personalised
- How you safeguard your patients
- How you ensure you have the right staff with the right skills
It might be helpful for you to start thinking now about how you meet the essential standards, what evidence you have that shows the outcomes your patients experience against the standards, and where you have gaps in that information.
Primary medical services that don’t meet all the standards on 1 April 2013 will still be able to register with us. However, you will need to tell us when you will be compliant and how, and/or how you are managing any associated risks.
The legislation does give us enforcement powers, which can lead to prosecution or fines, but again, we will only use these in serious cases of non-compliance that affect patient safety, and where a provider is unresponsive to improving the issues.
Once you are registered, we will monitor your compliance with the essential standards of quality and safety, focusing on areas where we think there is a risk that standards are not being met.

Most Practices have clinical systems that work well for them. Many have patient facing websites that work well for patients and the Practice.
The missing link in many GP surgeries is an efficient way of running the operational side of the Practice. Think of the operational elements of the Practice as a necessary chore for you and your staff that detracts from patient facing support functions. Then, relating this to cost, ask yourself the following question:
How efficient would your best employees be if you halved their pay after one year?
Wouldn’t it be good to have an employee who ensures all of your documents are current and simple to find, and can organize all of your evidence for all of your regulatory processes? If that employee made sure everyone reads mandatory documents and communicated all information across the Practice and Consortium, how much would they be worth?
As an added bonus and without claiming overtime, what if they could also make sure your address books were up to date and easily searchable? Or if they could control emergency drugs and fridge temperature logs and look after your staff rota? And populate staff calendars with training, appraisals, CRB checks, HepB vaccinations and take care of room bookings? Then control Practice assets, look after your complaints and significant events processes, organize meetings and tag all of your processes and documents for CQC registration and future inspection?
Then what if the same employee could control who can view, add, edit or delete any information within the operation of the Practice?
If you could employ someone to do all of this and more, at minimum wage, working for just 6.5 hours per week; then halve their pay after one year (whilst expecting them to do more), would you employ them?
Of course you would.
(Oh, and by the way, what if they never take sick leave or holidays, are never late for work, never forget tasks and work 7 days a week, 24 hours a day for no extra pay?)

In brief: CQC visits are starting up for GP organisations. Westcroft Health Centre and Novus have had CQC inspections in the last month. Intradoc247 has helped them get through as painlessly as possible.
Release:
Most practices now have their Care Quality Commission inspection date. A poor outcome from inspection can lead to the practice being closed down. This last month, both Westcroft Health Centre and Novus Health have had CQC visits. These can be painful affairs, venturing into the unknown, but fortunately both had Intradoc247 installed.
Gloria Bailey at Westcroft Health Centre had this to say: “The CQC assessors came to the practice on 8th Feb. We were able to show them, very quickly and easily with the aid of Intradoc247, how our policies and guidelines were stored and made accessible to all staff, and how review dates were recorded, and documentation distribution was logged. They were impressed at how easily, and accurately the information could be made available and linked directly to QoF and CQC. Also our staff training logs are able to be up-to-date and mandatory training needs identified.”

There is a good deal of change taking place in Business Continuity due to a number of key factors.
- Businesses have cut resources and staff to the minimum, so their dependency on fewer staff and systems is increasing the exposure to incidents and risks. When business goes off track there is less flexibility and resilience to be able to recover, so effective planning for dependencies, points of failure and resources is vital.
- Insurers are looking for better returns due to flat premiums over the past few years, but in particular to seek out better operational management and risk prevention within clients to reduce the impact and length of potential pay-outs. Business Continuity is a key part of this process.
- The message of taking better care of one’s own business is getting out there, and more businesses in the mid size range are turning to Business Continuity and Risk Management to secure cost savings and process improvement.
- Company executives are more aware that a failure to protect the business and the staff within it are now punishable with personal liabilities and cannot hide behind the company to protect themselves. Continuity planning is part of the expected protection of a business from collapse.
- Many large businesses are moving away from the use of large scale software deployments and are turning to more pragmatic and document oriented capabilities. Not only are these cheaper, but are far lighter on resource, training, consulting and re-learning costs which large semi-automated systems demand.
- The introduction of the ISO22301 Standard on “Societal” business continuity is improving the approach to the area in businesses. Senior management have to demonstrate ownership and involvement. Plan testing has to take place. Evidence of management, audit and improvement must be gathered.
- Sectors of public facing services are more inclined to go through the process and to develop plans – Academies, General Practice and Universities are amongst those areas becoming more aware that the failure to provide services – regardless of the cause – will severely impact reputation and funding.

On 17th May 2012, the Kingsgate Conference Centre in Peterborough will be host to the 4th Annual SNUG Conference.
This day long conference will bring together SystmOne users from a variety of settings, TPP, 3rd party suppliers and central policy makers to discuss, collaborate and explore the ongoing roles and future opportunities for SystemOne to contribute to patients’ health care needs in the ever changing NHS landscape.
Last year’s event was a great success and we have enhanced the programme this year to take account of feedback and new developements in strategy and focus with the aim of helping users make maximum use of their SystmOne clinical systems.
SystmOne has the 2nd largest GP system footprint in use throughout England and Wales and our aim is to use the success of previous SNUG conferences to provide a springboard for this year’s gathering to raise the profile, influence and membership of the National Usergroup to facilitate the better represention of our members and fellow SystmOne users at all levels of healthcare informatics negotiations and policy decision making.
